To file a claim you will need copies of your DD-Form 214, Service Medical Record/Service Treatment Record (if available) and any private medical records (if any outside care completed). You can request a copy of your DD Form 214, medical and service records through the National Archives and Records Administration's (NARA) website at https://www.archives.gov/veterans. From personal experience expect between eight to ten weeks before you receive them.
